The primary extracardiac inferior cavopulmonary connection is an unusual novel palliation for single-ventricle physiology, which we first performed in the setting of unfavourable upper-body systemic venous anatomy for a standard bi-directional Glenn, and in lieu of leaving our patient with shunt-dependent physiology. After an initial 16-month satisfactory follow-up, increasing cyanosis led to the discovery of a veno-venous collateral that was coiled, but, more importantly, to impressive growth of a previously diminutive superior caval vein, which allowed us to perform completion Fontan with a good outcome. Performing the single-ventricle staging in a reverse manner, first from below with a primary inferior cavopulmonary connection, followed by Fontan completion from above with a standard superior caval vein bi-directional Glenn, is also possible when deemed necessary.

This article reports on a qualitative research study that explored the perspectives and lived experiences of children in a range of New Zealand rural environments. Thirty-six children, aged between 6 and 11 years, were interviewed about living in the country and also contributed artwork and photographs. They came from four specific rural locations, ranging from ‘rural with high urban influence’ to ‘highly rural/remote’. Children expressed positive views about aspects of rural living, such as opportunities for being outdoors and participating in social relationships, confirming a positive discourse of the rural idyll. Their accounts highlighted children's agency under complex and sometimes challenging conditions. Children also, however, experienced some aspects of rural life as dull, dangerous or difficult. The complex and nuanced constructions of rural childhood uncovered in this study point to the critical importance of consulting with children in order to understand their experiences and best meet the needs of rural children and families.

Introduction
Debilitating patient-related non-cardiac co-morbidity cumulatively increases risk for congenital heart surgery. At our emerging programme, flexible surgical strategies were used in high-risk neonates and infants generally considered in-operable, in an attempt to make them surgical candidates and achieve excellent outcomes.
Materials and methodsBetween April, 2010 and November, 2013, all referred neonates (142) and infants (300) (average scores: RACHS 2.8 and STAT 3.0) underwent 442 primary cardiac operations: patients with bi-ventricular lesions underwent standard (n=294) or alternative (n=19) repair/staging strategies, such as pulmonary artery banding(s), ductal stenting, right outflow patching, etc. Patients with uni-ventricular hearts followed standard (n=96) or alternative hybrid (n=34) staging. The impact of major pre-operative risk factors (37%), standard or alternative surgical strategy, prematurity (50%), gestational age, low birth weight, genetic syndromes (23%), and major non-cardiac co-morbidity requiring same admission surgery (27%) was analysed on the need for extracorporeal membrane oxygenation, mortality, length of intubation, as well as ICU and hospital length of stays.
ResultsThe need for extracorporeal membrane oxygenation (8%) and hospital survival (94%) varied significantly between surgical strategy groups (p=0.0083 and 0.028, respectively). In high-risk patients, alternative bi- and uni-ventricular strategies minimised mortality, but were associated with prolonged intubation and ICU stay. Major pre-operative risk factors and lower weight at surgery significantly correlated with prolonged intubation, hospital length of stay, and mortality.
DiscussionIn our emerging programme, flexible surgical strategies were offered to 53/442 high-risk neonates and infants with complex CHDs and significant non-cardiac co-morbidity, in order to buffer risk and achieve patient survival, although at the cost of increased resource utilisation.

The superior cavopulmonary anastomosis – bi-directional Glenn – is the standard palliation for single ventricle physiology. When upper body systemic venous anatomic concerns such as superior caval vein stenosis, hypoplasia, or inadequate collateral tributaries are present, a Glenn may be precluded or have a high risk of poor outcome. A primary inferior cavopulmonary connection with an extracardiac conduit is an alternative palliation that provides a generous pathway for pulmonary blood flow, with the additional benefit of including hepatic venous return. We report a case of primary extracardiac inferior cavopulmonary connection in a patient unsuitable for Glenn, with successful post-operative outcome and early follow-up.

Euclid is the next ESA mission devoted to cosmology. It aims at observing most of the extragalactic sky, studying both gravitational lensing and clustering over ~15,000 square degrees. The mission is expected to be launched in year 2020 and to last six years. The sheer amount of data of different kinds, the variety of (un)known systematic effects and the complexity of measures require efforts both in sophisticated simulations and techniques of data analysis. We review the mission main characteristics, some aspects of the the survey and highlight some of the areas of interest to this meeting.

The behaviour of six species of African schistosomes and their hybrids has been compared qualitatively and quantitatively in baboons, with a view to finding a schistosome of high infectivity but low pathogenicity to the baboon for use in heterologous immunity experiments. This study, which includes the first comparative study of the pathogenicity of these six species in primates, is complementary to earlier studies in rodents in which these schistosomes and their hybrids were compared in terms of egg morphology, snail infectivity, and infectivity and pathogenicity. In rodents, statistically significant differences were detected in the behaviour of each pair of sibling species with regard to percentage worm recovery, rate of egg production and percentage distribution of the eggs in the tissues, and the hybrids also differed significantly from their “parental” species.
Even greater differences were found in baboons. For example, although all the strains studied produced mature egg-laying adults in rodents, no worms, eggs, or histopathological changes resulted from exposure of baboons to S. rodhaini or S. bovis. With regard to prepatent period, S. mansoni was the shortest (5–6 weeks), S. haematobium the longest (12–14 weeks), while S. mattheei and S. intercalatum occupied intermediate positions (7–9 weeks). The hybrids all matured in 7–9 weeks except the S. bovis/S. mattheei hybrid (11–16 weeks). S. haematobium similarly had a much longer maturation time in rodents. Complete or partial “self-cure” occurred in 3 baboons but was not observed in rodents. The percentage worm recovery of S. mansoni was higher than for the S. mansoni/S. rodhaini hybrids while with the S. haematobium group of species, worm recovery was highest with S. mattheei followed by S. intercalatum and S. haematobium, the same order as in rodents. With the S. haematobium-group hybrids, worm recovery was generally high except with the S. bovis/S. haematobiuun hybrid.

Background
Schizophrenia has been linked with psychological problems in childhood but there is little information on precursors of affective psychosis.
AimsTo compare childhood psychological antecedents of adult schizophrenia and affective psychosis.
MethodChildhood item sheets, which give standardised information on signs and symptoms of mental illness in the year preceding assessment are completed for all attendees at the children's department of the Maudsley and Bethlem Royal Hospital. We examined item sheet data on individuals with an adult diagnosis of schizophrenia (n=59) or affective psychosis (n=27) and a comparison group with no adult mental illness (n=86) (all had attended the department).
ResultsAbnormal suspiciousness or sensitivity and relationship difficulties with peers are associated with later schizophrenia. In contrast, affective psychosis is associated with childhood hysterical symptoms and disturbances in eating.
ConclusionsChildhood psychological precursors for schizophrenia and affective psychosis differ and do not simply reflect non-specific psychiatric disturbance in adolescence.

Species of Isoetes in northeastern North America may have evolved through hybridisation and allopolyploidy. Evidence for natural hybridisation was obtained from overlapping distribution patterns, abortive and polymorphic spore formation, chromosome numbers ranging from diploid to decaploid, and additive enzyme electrophoresis profiles. The ease with which hybrids form in culture supports the hypothesis that natural hybrids occur and that hybridisation could be involved in the evolution of new species.
